OVERVIEW OF SERVICES
Our initial session will concentrate on an assessment designed to establish a connection and build rapport while exploring your therapy needs and goals. I will employ a biopsychosocial approach, considering biological, psychological, and social factors, along with your demographics, symptoms, challenges, current functioning, family history, and personal aspirations.
*Parents are required to attend the initial session and will be required to make arrangements to check in every month.
Clients who complete their goals and/or fulfill their needs are always welcomed back, should they want to re-engage in services. Space will be available on a first-come, first-served basis, which may result in being placed on a waitlist.
MODALITIES
I use Cognitive Behavioral Therapy and ACT/ Acceptance and Commitment Therapy as my main form of treatment; however, I also use Trauma-focused Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Motivational Interviewing, solution-focused therapy, and a humanistic approach to support my clients. I also draw from Bowenian Therapy, Structural Family Therapy, and a few other modalities to ensure a well-rounded approach tailored to each individual.
COST
I offer 50-minute individual and joint sessions for children, pre-teens, adolescents, and adults. New patient intake and assessment sessions are $220, and subsequent appointments are $160. I also offer after-hours and weekend appointments at $220 per session on a case-by-case basis. I do not accept insurance; all payments are accepted via cash, HSA, or flex spending. I can, however, provide a superbill that you can send to your insurance provider for possible reimbursement. It is recommended that you check with your insurance provider regarding their reimbursement process and out-of-network benefits or coverage before scheduling, as you will be liable for any charges incurred. I also offer a sliding scale for those who qualify, which will be assessed on an individual basis. Payment is required before or at the time of service.
After-hours services are available at the rate of $220.00 for new intakes and $220.00 for ongoing sessions. After-hours services need to be talked about and scheduled with advance notice and require approval.
Please note: All miscellaneous tasks requiring 15 minutes or more will be charged a prorated rate of $50.00 per 15 Minutes. This includes consultation calls for established patients.
PAYMENT
Payment is collected through the account created when scheduling your appointments. It is essential to keep an updated card on file to ensure timely payment. Delinquent charges that exceed 30 days will incur a late fee of $50.00 per month. Please note: All refunds are subject to the payment processing fee associated with acuity.
SCHEDULING
Scheduled appointments should be made with the intention of meeting at the agreed-upon time and date, and with a readiness to engage. It is required that you schedule 8 weekly sessions after the initial intake if you plan to engage in services. This not only allows the patient to secure their space within the schedule but also creates consistency, which aids in the therapeutic process and demonstrates your commitment to services. Individuals may only schedule one appointment per week, unless prior arrangements have been made with the provider. No specific times or days of the week will be guaranteed. Available appointments will vary from early morning to afternoon, and some evenings. Individuals who consistently reschedule appointment times (regardless of notice), no-shows, give inadequate cancelation time (less than 48 hours), or frequently arrive late will be unable to schedule further sessions. Those who are inconsistent and fail to keep up with their scheduling will need to follow up to determine when and if services can be restored, as such behavior interferes with the therapeutic process. Please take a look at the cancellation policy for more information.
Please be considerate of others when scheduling and rescheduling, as frequent rescheduling can create missed opportunities for others.
*For those scheduling multiple appointments, it is your sole responsibility to manage the scheduled times. Any missed sessions will be charged the full $160.00 or $220.00 rate.
DIVORCED/ SEPARATED PARENTS
Divorced or separating parents with legal guardianship must review and sign the divorce expectations before the time of service. Both parents must agree and sign before any service will be provided to the child. Both parents also need to review and sign the authorizations and acknowledgment before the session. Legal documentation will also be required for those who specify having sole legal guardianship.
Services can be provided to parents who are separated and unable to gain consent from the other parent; however, no direct care will be provided for the child.
All communication between divorced and/or separated parents or caregivers is the sole responsibility of the parents/caregivers and should be arranged before engaging in therapy.
COURT ORDERS/ LEGAL ADVISORY NOTICE
Rudy and Thrive Mental Health LLC will not engage in any legal or court-related matters. In the event of a subpoena or legal requirement, a fee of $2,500 will be applied. For each court appearance, the charge for any court-related matters is $1,000 per hour, applicable for any court involvement needed for you or your family.
I do not provide voluntary testimony. Additional fees include:
FEES FOR LEGAL INVOLVEMENT / COURT APPEARANCE
All attorney costs incurred are payable by the patient or the patient's responsible guardian.
Fees are due 7 days prior to your court date. They are non-refundable, as I will clear my schedule for the appearance. If the court date is rescheduled, you will incur the exact fees and costs associated with the canceled appointments. No new appointments will be scheduled until this fee is fully paid. Payment must be received within one calendar month to prevent further action.
SCHOOL SUPPORTS
School can be challenging, and for those experiencing additional challenges, it can be difficult to overcome without additional resources and support. Collaboration with school counselors and administrators can be assessed as needed by the Behavioral Health Specialists or Therapists to increase support for the client.
Request for 504 letters or letters of accommodation can be assessed, but are not guaranteed.
CANCELATION POLICY
Accidents happen and things come up, however, it is important that the time we agree upon is respected. Any missed session will be charged the full $160.00 or $220.00 rate. Appointments that are not canceled with 48 hours’ notice will also be charged the full rate of the session, $160.00 or $220.00. THIS IS NON-NEGOTIABLE. Please note: Scheduling is the sole responsibility of the client/caregiver. Any fees accrued due to cancellation or rescheduling will not be covered by insurance and will not be added to any superbill. Additionally, clients who arrive 10 minutes late to their scheduled appointment will be charged the full $160.00 or $220.00 rate of the session, and the appointment to which they were late will be canceled. Consistent late or canceled appointments will be evaluated and may result in the termination of services.
RESCHEDULING POLICY
Life happens, and it is often necessary to reschedule an appointment or two. Regarding the therapy process and the commitment to engage in services, it is necessary to communicate the need to reschedule at least 48 hours in advance. Appointments that are rescheduled with less than 48 hours’ notice will be charged the full $160.00 or $220.00 rate of the scheduled session. Individuals who consistently reschedule will lose the privilege of scheduling their own sessions.
A credit card will be required to secure any appointments. Please ensure that your card information is up to date and accurate.
Please be considerate of others when scheduling and rescheduling, as frequent rescheduling can create missed opportunities for others.
