OVERVIEW OF SERVICES

Our initial session will be an assessment. During the assessment, we will explore your needs and wants to engage in therapy. During this, I will utilize a biopsychosocial approach to complete the assessment where we will review demographics, symptoms, presenting challenges, family history, personal needs, and goals, and aid in the development of a treatment plan and potential diagnosis. Confidentiality, mandated reporting, Tarasoff or Duty to Warn, and patient responsibilities will also be covered in this session. Every subsequent session will be used to target the treatment plan and the needs and goals of the patient. Our sessions will be comprised of a beginning, where we will review the previous session and homework, if applicable, a middle portion, where we will do the majority of our work, and an ending, where we will review the session, review homework, and set an agenda for the next session.

*Parents are required to attend the initial session and will be required to make arrangements to check in on a regular basis.

Clients who complete their goals and/or fulfill their needs are always welcomed back should they want to re-engage in services. Space will be on a first come first serve basis, which could lead to being placed on a waitlist.

MODALITIES

I use Cognitive Behavioral Therapy as my main form of treatment, however, I also use Trauma-focused Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Motivational Interviewing, Solution Focused Therapy, and a humanistic approach to support my clients. I also pull from Bowenian Therapy, Structural Family Therapy, and a few other modalities to ensure a well-rounded approach suited for each individual.

I also offer after-hour and weekend appointments at $220 per session on a case-by-case basis. I do not accept insurance and all are cash pay, HSA, or flex spending. I can, however, provide a superbill that you can send to your insurance provider for possible reimbursement. It is recommended that you check with your insurance provider on their reimbursement process and out-of-network benefits/ coverage prior to scheduling as you will be liable for any charge you incur. I also offer a sliding scale for those who qualify, which will be assessed on an individual basis. Payment is required prior to or at the time of service.

Delinquent charges that go beyond 30 days will be charged a late fee of $50.00 per month the charge is late. Please note: All refunds are subject to the payment processing fee associated with acuity.

SCHEDULING

Scheduled appointments should be made with the intent to meet at the agreed-upon time and date, and readiness to engage. It is required that you schedule out 8 weekly sessions after the initial intake if you plan on engaging in services. This not only allows the patient to secure their space within the schedule but also creates consistency, which aids in the therapeutic process and demonstrates your commitment to services. Individuals may only schedule one appointment per week, unless prior arrangements have been made with the provider. No specific times or days of the week will be guaranteed. Available appointments will vary from early morning to afternoon, and some evenings. individuals who consistently reschedule appointment times (regardless of notice), no-shows, give inadequate cancelation time (less than 48 hours), or frequently arrive late will be unable to schedule further sessions. Those who are inconsistent and fail to keep up with scheduling will need to follow up to determine when and if services can be restored as such behavior interferes with the process of therapy. Please refer to the Cancelation Policy for more information.

*Please be considerate of others when scheduling and rescheduling as frequent rescheduling creates missed opportunities for others.

*For those scheduling multiple appointments it is your sole responsibility to manage the scheduled times. Any missed sessions will be charged the full $140.00 or $200.00 rate.

DIVORCED/ SEPARATED PARENTS

Divorced or separating parents with legal guardianship must review and sign the divorce expectations prior to the time of service. Both parents must agree and sign before any service will be provided to the child. Both parents also need to review and sign the authorizations and acknowledgment before the session. Legal documentation will also be needed for those who specify having sole legal guardianship.

Services can be provided to parents who are separated and unable to gain consent from the other parent, however, no direct care will be provided for the child.

All communication between divorced and/or separated parents, or caregivers is the sole responsibility of the parents/caregivers and should be arranged prior to engaging in therapy.

COURT ORDERS AND ASSESSMENTS

Court-ordered assessments and court-mandated clients will be evaluated on an individual basis, however, I will be unable to engage in custody evaluations or court proceedings.

SCHOOL SUPPORTS

School can be challenging and for those experiencing additional challenges that can be difficult to overcome without additional resources and support. Collaboration with school counselors/administrators can be assessed as seen fit by the Behavioral Health Specialists/Therapists to increase support for the client.

Request for 504 letters or letters of accommodation can be assessed but are not guaranteed.

CANCELATION POLICY

Accidents happen and things come up, however, it is important that the time we agree upon is respected. Any missed session will be charged the full $160.00 or $220.00 rate. Appointments that are not canceled with 48 hours’ notice will also be charged the full rate of the session $160.00 or $220.00 rate of the session. THIS IS NON-NEGOTIABLE. Please note: Scheduling is the sole responsibility of the client/ caregiver and any fees accrued due to cancelation or rescheduling will not be covered by insurance, and will not be added to any superbill. Additionally, clients who arrive 10 minutes late to their scheduled appointment will be charged the full $160.00 or $220.00 rate of the session, and the appointment to which they were late will be canceled. Consistent late or canceled appointments will be evaluated and may result in the termination of services.

RESCHEDULING POLICY

Life happens and it is often necessary to reschedule an appointment or two. With respect to the process of therapy, the clinician, and the commitment made to engage in services it is required that you communicate the need to reschedule and provide 48 hours before rescheduling any appointment. Appointments that are rescheduled with less than 48 hours notice will be charged the full $160.00 or $220.00 rate of the scheduled session. Individuals who display a consistent pattern of rescheduling will lose the privilege of scheduling their own sessions.

A credit card will be required to secure any appointments. Please ensure that your card is updated and that the information is accurate.

*Please be considerate of others when scheduling and rescheduling as frequent rescheduling creates missed opportunities for others.
